Evaluation of Impact of Tuberculosis Control Among Non-natives in Shenzhen City

Objective To bring the non-natives into the TB control program and try to find the effective model for decreasing the TB prevalence and increasing the cure rate,therefore to improve the efficacy of TB control within Shenzhen City.Methods The TB control program data of 1993 to 1997 and 1998 to 2004 were retrospectively reviewed,the former did not involve the non-natives,but the latter did.The efficacy between both periods was evaluated.Results The epidemic characteristics of TB in Shenzhen showed that the non-natives accounted for 72.36% of the total populationl,and 53.5% of new sputum testing positive cases were the non-natives.Most of the new sputum positive non-natives were the 20 to 30 years-old group,and the males were 2.153 times of the females.The sputum positive cases included factory workers,commercial service employees and the unemployed.The workers accounted for 33.4% of all the positive cases. After launching the non-natives TB control program,the transferred treatment cases increased from 9,727 to 61,653,and the accomplishment rate of transferred treatment increased from 63.5% to 90.2%,the differences were statistically significant(P0.005).Among the 1993-2004 newly diagnosed cases,88,798 were the non-natives,accounted for 80.62%.Among all the non-natives TB patients,9,496 of them were sputum positives and 4,845 negatives,positive rate was 69.5%.The completed supervising rates were 79.1% and 95.6%,and the completed management rates were 62.6% and 96.2% respectively, the differences were statistically significant(P0.005).At the end of 2-month treatment,the negative rate was increased from 63.3% to 84.6%,and at the end of 3-month treatment,from 71.9% to 92.2%,the differences were statistically significant(P0.005).Before non-natives involving into the program,the cure rate was only 87.2%,however,after being brought into the program,the cure rate for positive TB patients was reached to 97.2%,and the differences were statistically significant(P0.005).Conclusions The non-natives are the major component of TB patients in Shenzhen.Through promising and paying great attention to the establishment of the networks on three levels by the government,restricting ascription management,establishing the mechanism of supervision and evaluation,bringing the non-natives into TB control program,strengthening health education and propagation,the TB prevalence decreases annually,the cure rate increases constantly,and the TB control efficacy is improved in Shenzhen City.
