Extended Infrafrontal Approach to Resect Cranial Base Tumors: Report of 10 Cases

(Purpose) To introfuce an operative methed of extended inferior frontal antenor cranial basiler combined approach and report the microsurgical experience in 10 cases of huge, widely invadjng cranial base tumors. (Methods) After an obital- frontal craniotomy was made, the planum sphenoidale and the orbit-top were removed. And then through extra-dura and sub-dura space, tumor would be rearted by microneurosurgical technique. At last, recostruction of crania1 hase was made. (Results) Total removal was achieved in 5 cases, 3 cases were subtotal removal and 2 cases were partial removal. No one died of operation or suffered from serious complication. (Conclusions) The approach is fit for large, widely invading cranial base tumor, especialy in the midline. It could expose tumor widely and minimize brain injury. The reconstruction of cranial base is important to the success of operation.
