Sequential Chemotherapy and Boost Intensity Modulated Radiation Therapy for 31 Cases with Non-small Cell Lung Cancer Stage Ⅲ

[Purpose] To compare the efficacy and toxicity of sequential chemotherapy and boost intensity modulated radiation therapy(IMRT)with sequential chemotherapy and conventional radiotherapy for non-small cell lung cancer(NSCLC)stage Ⅲ.[Methods] Eighty-seven cases of stage Ⅲ non-small cell lung cancer were divided into 2 groups:group A and group B.Thirty-one cases of group A were treated with sequential vinorelbine and cisplatin regimen 1～2 cycles,conventional radiotherapy 40Gy/20f,4W,and boost IMRT with a total dose of(70～74)Gy/(35～37)f,and after radiotherapy chemotherapy to be continued.Fifty-six cases of group B were treated with sequential vinorelbine and cisplatin regimen and conventional radiotherapy with a total dose of(60～64)Gy/(30～32)f,(6～7)W.[Results] Eighty-seven cases followed up for 24 to 36 months.The overall response rate,1-and 2-year survival rates in group A and group B were 80.65% vs.76.56%,51.61% vs.35.71% and 35.48% vs.17.86% respectively with no significant difference.The radiation esophagitis,radiation pneumonitis and myelosuppression in group A and group B were 58.06% vs.79.64%,6.45% vs.16.07% and 54.83% vs.85.71% respectively with no significant difference.[Conclusions] The efficacy of sequential chemotherapy and boost IMRT is close to conventional sequential radiotherapy and chemotherapy for NSCLC without increasing toxicity.
