An Analysis of Locoregional Recurrent Sites in Patients with Nasopharyngeal Carcinoma After Different External Beam Radiotherapy

[Purpose] To analyze the characteristic of locoregional recurrent sites in patients with nasopharyngeal carcinoma(NPC) after different external beam irradiation, and to provide some suggestions for the clinical practice. [Methods] One hundred thirty-one locoregional recurrent NPC patients were divided into three groups (traditional radiotherapy, conventional radiotherapy, three dimendional conformal radiation therapy and intensity modulated radiation therapy (3D-CRT and IMRI))according to the external irradiation teniques from January 2002 to December 2004. Locoregional recurrent sites were analyzed and compared among three groups.[Results] Recurrence in carotid sheath in conventional radiotherapy group was lower than that in traditional one(32.4% vs. 52.5%,P0.05), and no difference was found in nasopharyngeal cavity, skull base, superior cervical lymph node recurrence between the two groups (P0.05). Six patients developed local recurrence and 3 patients developed regional lymph node recurrence in 3D-CRT and IMRT group. [Conclusion] The main locoregional recurrent sites were carotid sheath, cervical and/or skull base in the patients with NPC after radiation therapy. Conventional radiotherapy meets the clinical fetures and treatment dosimetric principles of NPC and reveals certain advantages comparing with traditional one . 3D-CRT and IMRT have potential advantage over conventional and traditional ones, but need further confirmation.
