Predictive Factors Related to Metastasis of Non-sentinel Lymph Nodes in Early Breast Cancer Patients with Positive Sentinel Lymph Nodes

[Objective] To investigate the predictive factors related to the metastasis of non-sentinel lymph nodes(NSLN) in early breast cancer patients with positive sentinel lymph nodes(SLN).[Methods] The clinical data of 578 female breast cancer patients with positive SLN admitted in Henan Tumor Hospital from July 2010 to August 2017 were analyzed retrospectively. Touch imprint cytology and serial sectioning with HE staining were routinely performed on each sentinel node. The factors related to the metastases of NSLN were analyzed with univariate and multivariate Logistic regression analysis. [Results] Positive NSLN were found in 38.4% of 578 patients.Univariate analysis showed that the number of positive SLN(χ~2=70.114,P0.001),the number of negative SLN(χ~2=49.095, P0.001),and expression level of Ki67(χ~2=6.924,P=0.009) were associated with NSLN metastasis. Multivariate analysis showed that the number of positive SLN(OR=2.076,95% CI:1.686 ～2.556,P 0.001),the number of negative SLN(OR =0.673,95% CI:0.586 ～0.773,P0.001),and the expression level of Ki67(OR=1.807,95%CI:1.150～2.840,P=0.010) were independent predictors of NSLN metastasis. [Conclusion] Number of positive SLN,number of negative SLN and the expression level of Ki67 are predictive factors of NSLN metastasis in breast cancer patients with positive SLN.
