Primary research on clinical value of sCD40L in early diagnosis and prognostic assessment of colorectal cancer

Objective To investigate the clinical value of soluble CD40 ligand(sCD40L) in the early diagnosis of patients with colorectal cancer and their prognostic assessment. Methods The serum s CD40 L and carcinoembryonic antigen(CEA) concentration were detected by Luminex x MAP analysis and ELFA method in 78 patients with colorectal cancer and 95 healthy control people.ROC curve was used to make comparison between the clinical value of s CD40 L and CEA in the diagnosis of colorectal cancer. Those patients with colorectal cancer were divided into s CD40 L high expression group(sCD40Lhigh) and s CD40 L low expression group(sCD40Llow). Analysis was made in the correlation between serum s CD40 L level and clinical pathological parameters. Kaplan-Meier survival curve was used to analyze the effects of s CD40 L expression level on patients' prognosis. Results The serum s CD40 L concentration of patients with colorectal cancer was significantly higher than that of healthy controls. ROC curve showed that the specificity and sensitivity of s CD40 L in the diagnosis and prognosis of colorectal cancer was superior to those of CEA, and the AUC value of s CD40 L was larger than that of CEA. The concentration of serum s CD40 L was closely correlated with lymph node metastasis,the depth of tumor invasion, and TNM staging. The survival time of patients with high s CD40 L expression level was significantly shorter than that of those in the low expression group. Reexamination showed the significant decreasing of serum s CD40 L concentration in 15 cases one year after the operation. Conclusion SCD40 L has certain value in the evaluation of the early diagnosis and prognosis of patients with colorectal cancer.
