Functional Outcome after Arthroscopic Rotator Cuff Repair:Double-Row Versus Suture-Bridge Technique

Background Rotator cuff tears is a common shoulder disease among the elderly. Withthe improvement of arthroscopic technique,more and more patients with rotator cuff tears have receivedthe arthroscopic rotator cuff repair. Only a few studies have examined the functional outcome betweenthe suture-bridge and double-row techniques. Objective To compare the functional outcome and repairintegrity of arthroscopic double-row and conventional suture-bridge repair in full-thickness rotator cufftears. Methods Forty consecutive full-thickness rotator cuff tears between October 2014 and 2015 un-derwent arthroscopic rotator cuff repair at our hospital were included:20 patients were treated usingthe double-row technique and the other 20 were done using the suture-bridge technique. Clinical andimaging outcomes were evaluated using the University of California at Los Angeles(UCLA),Americanshoulder and elbow surgeons(ASES),constant score,visual analogue scale(VAS),range of motion(ROM)and magnetic resonance imaging(MRI). The patients' satisfaction,operation duration and hospital costwere compared between the two groups. Results At the final follow-up(six months postoperatively),theaverage UCLA,ASES and constant score improved significantly to 33.1,89.4 and 83.5 in the double-row group and to 31.85,84.6 and 82.4 in the suture-bridge group compared to those before the opera-tion,without significant differences between the two groups(P0.001). Both groups showed significant im-provement in ROM after surgery(P0.001),but there was no significant differences between them atthe final follow-up(P=0.986 and 0.323 at forward flexion and external abduction respectively). The av-erage VAS reduced significantly to 2.15 and 1.85 respectively(P0.001 for both),without significant dif-ference between the 2 groups at the final follow-up(P=0.389). Most patients were very satisfied withthe efficacy,with 2 patients in the double-row group and 1 in the suture-bridge group thinking of theresults as so-so. The operation time was 107(75-145)min and 92(65-125)min respectively in the dou-ble-row and suture-bridge groups,and their hospital cost was 42980 RMB and 53908 RMB respective-ly. The re-tearing was found in 2 patients of the double-row group and 1 of the suture-bridge groupthrough MRI. Conclusions Arthroscopic rotator cuff repair can get significant results in patients under-going full-thickness rotator cuff tears either using double-row or suture-bridge technique without signifi-cant differences.
