Application of end tidal carbon dioxide tension monitoring acute exacerbation of chronic obstructive pulmonary disease patients

Objective To study the correlation between end tidal carbon dioxide partial pressure and arterial carbon dioxide partial pressure in patients with acute exacerbation of chronic obstructive pulmonary disease,and to evaluate its clinical significance.Methods A total of 40 patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD) and 29 normal adults took pulmonary function tests,including FEV_1,FEV_1/FVC,MMEF,RV/TLC and sRAW.The values of end tidal carbon dioxide tension(PetCO_2) and arterial carbon dioxide tension(PaCO_2) were measured at the same time,and then the value of ADCO_2 was calculated.Results(1)The FEV_1,FEV_1/FVC and MMEF values of AECOPD patients were significantly decreased(t =4.52,t =4.22,t =3.80,P =0.039,P =0.027,P =0.021),and the values of sRAW and RV/TLC were significandy increased(t =6.31,t = 1.73,P =0.041,P =0.0087).(2) Compared with normal control group,the value of PaCO_2 in AECOPD group increased significantly(t =2.11,P =0.034).The value of PetCO_2 in AECOPD group were significantly lower than this in normal control group(t = 1.83,P =0.029).PetCO_2 in both of the groups had significant correlation with PaCO_2(r =0.96,r = 0.63,all P 0.01).ADCO_2 in AECOPD group was significantly associated with FEV_1,MMEF,SRAW and RV/TLC(P 0.05).Conclusion The PetCO_2 and PaCO_2 have a good correlation,PetCO_2 can be used to judge the change of PaCO_2 in normal and AECOPD patients,and the status of respiratory function in patients with AECOPD.ADCO_2 reflects the obstruction degree of ventilation,and may be used to evaluate the severity of AECOPD.
