Effects of calcitriol on elderly patients with type 2 diabetes and its influence on inflammatory factors

Objective To observe the effect of calcitriol on elderly patients with type 2 diabetes mellitus.Methods Elderly patients with type 2 diabetes who were vitamin D insufficiency or deficiency were randomly divided into two groups.50 cases in each group.The observation group received oral calcitirol,and the control group was not given any drugs.Both groups were treated for 12 weeks.The changes of blood pressure,BMI,fasting blood glucose,HOMA index,blood lipid,HbA_(1c),high sensitive C reactive protein,homocysteine and carotid intima media thickness were observed in the two groups before and after treatment.Results Compared with control group,after treatment,diastolic blood pressure,fasting blood glucose,HbAlc,low density lipoprotein cholesterol,hs-CRP and HOMA-IR were all decreased in observation group,and the difference were statistically significant(t =-2.172,t=-2.008,t=-2.068,t =-2.157,t =-2.344,t=-2.050;P =0.032,P =0.047,P =0.041,P =0.033,P =0.021,P =0.043).After treatment,25(OH)D,FINS in the observation group increased,the difference were statistically significant(t =3.869,t =2.288,P =0.000,P =0.024).There were no significant changes in body mass index,TG,total cholesterol,HDL-C,homocysteine,carotid intima media thickness before and after treatment(t =-0.169,t=-0.905,t=-0.501,t=1.379,t=-1.055,t=-0.479;P=0.866,P=0.368,P =0.618,P =0.171,P =0.294,P =0.633).25(OH) D was negatively correlated with FPG,HbA_(1c),LDL-C,HOMA-IR and CRP(r=-0.363,r =-0.326,r =-0.059,r =-0.345,r =-0.145,P =0.033,P =0.035,P =0.043,P =0.022,P =0.042),25(OH)D was positively correlated with fasting insulin(r =0.318,P =0.025).25(OH) D was not associated with SBP(r =-0.067,P =0.065).Conclusion Calcitriol can improve insulin resistance,regulate blood glucose,blood lipid,blood pressure,reduce hs-CRP.
