Differenciation of necrotic brain tumors and brain abscess using DWI imaging

Objective To discuss the value of DWI in differentiation of necrotic brain tumors and brain abscess. Methods 24 cases of brain tumors or brain abscess proved by operation or clinical diagnosis were retrospectively analyzed. All these lesions had liquefied area on the routine MR,for which the value of ADC( apparent diffusion coefficient) was measured and statistical analysis performed. Results Among the 24 cases of brain diseases, 10 cases were necrotic gliomas,6 cases were necrotic metastatic tumor and 8 were brain abscess. The signal of Abscess cavity was high on DWI,and the average ADC value was( 0. 42 ± 0. 12) × 10- 3mm2/ s. While the signal of necrotic brain tumors was low on DWI,and the average ADC value was( 2. 33 ± 0. 36) × 10- 3mm2/ s. The difference between ADC value of cystic compontions of brain tumors and brain abscess was statistically significant( t =6. 652,P 0. 001). Conclusion Both brain tumor and brain abscess may have liquefied area. It is difficult to distinguish them on routine MRI,while DWI can distinguish them accurately.
