Tubal and paratubal borderline tumors:a report of five cases and literature review

Objective To investigate the clinicopathological features of the borderline tumors located in the fallopian tube or mesosalpinx.Methods We retrospectively reviewed the clinical and pathological data of 5 patients admitted in our hospital and summarized the features of 21 cases reported in the literature.Results The 26 patients ranged from 3 to 75(average 36.1) years of age.The main clinical manifestations were lower abdominal pain(30.8%) and pelvic mass(84%).Ultrasound examination usually showed adnexal cystic mass with internal papillations and the serum CAl25 levels were normal(35 U/ml) or only mildly elevated(mean 89.9U/ml).Most of those cases(96.2%,25/26) had the lesion confined to unilateral fallopion tube or mesosalpinx.22 cases were cystic tumors,of which 73% had internal papillations.Another 3 were solid.Most of the tumors were medium size with the average diameter of 5.8 cm,measuring from 1.7 cm~13 cm.Histologically,serous tumors accounted for the majority(69.2%).All patients were treated exclusively by surgery: 2 cases had no available information,16 cases underwent fertility-sparing surgery,of whom 13 received conservative surgeries including tumor resection,partial or complete salpingectomy and unilateral salpingectomy.Hysterectomy with bilateral salpingo-oophorectomy was performed in 8 patients.There were 2 cases in all underwent complete staging/restaging operation.The patients were followed up for 6 to 84 months after surgery and all survived without recurrence.Conclusion Borderline tubal or paratubal tumors were rare and commonly occurred in young women of reproductive age.It was difficult to diagnose prior to the surgical exploration.Definitive diagnosis depended on pathological examination.The patients were usually at early stage when diagnosed and had good clinical outcomes.Preservation of reproductive ability by a conservative operation was safe and optimal for young patients.The significance of complete staging/restaging surgery is still not very clear.Long term follow-up and more cases are needed to make definitive conclusions.
