DETECTION OF PML-RARα FUSION GENE AND MNMALRESIDUAL LEUKEMC CELLS WITH RT/PCR TO PREDICTTHE PROGNOSIS OF ACUTE PROMYELCOYTICLEUKEMIA

his study performed in 97 cases of Chinese APL,including 35 cases at diagnosis, 44 cases during CR and18 cases both at diagnosis and during CR. The twomajor PML-RARa fusion mRNA isoforms of long (L) orshort (S) type were detected in 52 cases at diagnosis.There were more early deaths during the all-transretinoic acid (ATRA) induction treatment and morerelapses with in 2 years of CR in the S type (6/19cases) than in the L type group (2/33 cases). Thisdifference in overall prognosis was statistically significant(P 0. 025). 11 out of 62 APL patients investigatedduring CR exhibited positive RT/PCR. 5 of themrelapsed from 1 to 6 months after the test, oneconverted to negative after two cour-ses ofchemotherapy, 5 remained in CR status without furtherPCR data. However, the latter 5 cases all receivedintensive consolidation therapy after the PCR po-sitivity. These results suggested that RT/PCR could beused to monitor MRD in APL and that a positive RT/PCR of PML-RARα is an indicator of relapse in APL.
