Comparison of Clinical Characteristic of Early and Late Ovarian Stimulation Syndrome

【Objective】 To study the difference in clinical characteristic of early and late ovarian hyperstimulation syndrome(OHSS) 【Methods】 From July 2008 to July 2010,91 women were hospitalized due to moderate to severe OHSS after controlled ovarian hyperstimulation(COH) for IVF or ICSI.The OHSS was classified as the late type(n = 28) if it occurred 10 or more days after oocyte retrieval,and early type(n = 63) if it occurred within 9 days.Patient's characteristics,outcomes of COH and pregnancy were compared between the early and late OHSS groups and between the moderate and severe subgroups in early OHSS group.【Results】 The moderate and severe OHSS rate during that time was 1.9%.There were no statistically significant differences between early and late OHSS groups in terms of female age,BMI,PCOS history,oocyte retrieved,proportion of severe cases and multiple pregnancy rate.Differences in terms of clinical pregnancy rate(69.4% vs 100% P = 0.01),basal LH:FSH(0.91 ± 0.49 vs 0.71 ± 0.20 P = 0.006),proportion of cases with serum E2 on HCG day ≥ 5 000 pg/ml(33.3 % vs 7.1%,P=0.008) and using HCG as luteal support(47.6% vs 21.4% P = 0.018) were significant between early and late OHSS groups and were not significant between moderate and severe subgroups in early OHSS group.【Conclusions】 Basal LH:FSH,serum E2 level on HCG day and luteal support with HCG were associated with the time regarding the onset of OHSS.Our findings support that early OHSS was induced by exogenous HCG and late OHSS by exogenous HCG.
