A comparative study on gut barrier function damage following multiple and one-site firearm trauma in a porcine model

Objective To discuss whether multiple firearm trauma leads to more significant gut barrier function damage than one site trauma. Methods High speed 5 8 mm bullets were used to induce various injuries. 24 small pigs were randomly divided into 4 groups. Control group (Group C, n=6) was subjected to catheterization only,head injury group(Group H, n=6) to tangential fracture of parietal bone, comminuted fracture group (Group L,n=6) to comminuted fractures of bilateral femora. Multiple trauma group (Group M, n=6) to multiple injuries including both injuries of group H and group L(ISS≥16). Plasma concentrations of D lactate, diamine oxidase (DAO) and endotoxin in portal blood were detected before and after trauma. The animals were sacrificed at 72 h after trauma, and tissue concentration of DAO in small bowel was detected and the pathologic changes of intestine were observed. Results There were no remarkable changes of all above mentioned parameters in control group. As compared with one site injury groups (Group H and Group L), the animals in multiple trauma group had higher concentration of serum D lactate (P 0 01 at 48 h and 72 h,vs group H), higher contents of serum DAO till 72h while those in Group H and Group L had returned to normal level, lower content of intestinal tissue DAO, significantly higher serum endotoxin level(P 0 05~0 01 at 72h), and more severe pathologic changes of intestine (necrosis and exfoliation at ileum villus top). Conclusions Multiple firearm trauma leads to more significant gut barrier function damage compared with one site trauma in pigs, which may cause endotoxin translocation.
