Morphological characteristics of anterior visual pathway and the relationship between morphological changes and retinal nerve fibre layer thickness in patients with primary open-angle glaucoma

Objective To explore the morphological characteristics of optic tracts and optic chiasma in primary open-angle glaucoma (POAG) patients, and to analyze the relationship between the morphologic changes and retinal nerve fibre layer thickness (RNFLT). Methods Brain MRI was performed in 37 POAG patients (POAG group) and 1000 healthy volunteers (control group) with T1 weighted three-dimensional magnetization prepared rapid acquisition gradient echo (T1W-3D-MP RAGE) sequence. All data of optic tracts and optic chiasma were obtained with multibaseline and multiplanar reconstruction. The diameters of optic tracts and optic chiasma were measured, including the transverse diameter (TD1), the height (H1) of cisternal optic tract, the transverse diameter (TD2) and the height (H2) of peri-crural optic tract, the transverse diameter (OC-TD), the height (OC-H) and the anteroposterior diameter (APD) of optic chiasma. The two sample-t-test was done to compare the diameters between the two groups, while Pearson linear correlation analysis was performed to study the correlation between the diameters and RNFLT. Results ①The measured values of bilateral TD1, H1, TD2 and H2 in POAG group were lower than those of control group (all P0.05). ②The OC-TD and OC-H of POAG group were lower than those of control group (all P0.05), while there was no statistical difference of APD between the two groups. ③There was positive correlation between the sum of bilateral TD1, H1, TD2, H2 and the sum of binocular RNFLT in POAG group (r=0.77, 0.77, 0.81 and 0.77, all P0.05). ④The sum of binocular RNFLT correlated positively with OC-TD and OC-H (r=0.44 and 0.60, all P0.05), while no correlation was found between APD and the sum of binocular RNFLT (r=0.10, P0.05). Conclusion The atrophy of optic tract and optic chiasma is related to the defect of RNFLT in POAG patients.
