MR diffusion tensor imaging in diagnosis of chronic cervical spondylotic myelopathy

Objective To observe the value of DTI in diagnosis of chronic cervical spondylotic myelopathy with 3.0T MR.Methods DTI were performed in 64 patients with chronic cervical spondylotic myelopathy exceeded two years(CSM group) and 31 consecutive healthy subjects(control group) using SE-EPI sequence on the cervical spinal cord.The cervical cord was divided into the upper cervical cord,the mid cervical cord and the lower cervical cord according to anatomic structures.ADC,FA,longitudinal eigenvalue(λL) and horizontal eigenvalue(λT) of every segment cervical cord were measures and analyzed.Results High quality ADC maps and FA maps were obtained in all subjects.In CSM group,significant differences of ADC(F=251.20),FA(F=47.54),λL(F=210.14) and λT(F=104.39) values were found among 3 cervical cord regions(all P0.0001).In control group,the average ADC,FA,λL and λT values were(0.81±0.06)×10-3 mm2/s,0.65±0.04,(1.66±0.10)×10-3 mm2/s and(0.47±0.04)×10-3 mm2/s,respectively,no significant difference was found among 3 cervical cord regions.There were significant differences of ADC(t=20.84),FA(t=12.53),λL(t=6.00) and λT(t=20.84) values between CSM group and the control group(P0.05).Conclusion ADC,FA,λL and λT values are sensitive to observe changes of cervical spinal cord.DTI is useful to reflect the sites and severity of cord injury in chronic cervical spondylosis patients.
